Job Overview-

The Advanced Practice Nurse Practitioner (APN) for the Cardiology Service at Rahway works collaboratively within a multidisciplinary health team and is responsible for providing comprehensive and age appropriate direct care and follow up to a population of patients. The components of the role include clinical practice, consultation, professional development, education, and research. The APN works in conjunction with the Cardiology Attending performing patient consults and follow up care. Clinical core privileges will include the management, treatment, and ordering, performing, and interpreting labs, radiography, and other diagnostics test for acute and chronic cardiac disorders in the adult population.

Qualifications:

Required:

  • MSN/DNP from an accredited nursing program required
  • Active New Jersey RN and APN license
  • 1-2 years of acute care experience.
  • At least one year of APN experience.

Preferred:

  • The ideal candidate has at least three (3) years of acute care work experience
  • Experience in cardiology

Scheduling Requirements:

  • Per Diem
  • Scheduled as needed

Essential Functions:

  • Utilizes advanced assessment skills to manage a cohort of patients. Performs and documents assessments.
  • Collects and analyzes data and formulates a patient-centered plan of care.
  • Evaluates and monitors the effectiveness of the prescribed treatment plan.
  • Using the most current evidence modifies the plan of care based on the effectiveness of the treatment and the patient/family response to the plan.
  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
  • Serves as an educational resource to the patient/family, nursing staff, and provider team.
  • Acts as a consultant to multidisciplinary health care team members.
  • Educates patients/families about self-management of acute or chronic illnesses, tailoring instruction s to patients individual circumstance.
  • Facilitates the patient s transition within and between health care settings admitting, transferring, and discharging patients when appropriate.
  • Demonstrates a commitment to patient safety by speaking up about safety issues and changing practice to enhance safety. Shares errors or potential errors through system reporting mechanisms.
  • Demonstrates shared accountability for safe practice.
    Establishes and maintains communication with other health care providers and departments to ensure operational efficiency and effectiveness and to promote consistency and continuity of care.
  • Participates in scholarly activities including research and education.
  • Participates in self motivated professional development by maintaining required competencies, identifying learning needs and seeking appropriate educational opportunities.
  • Participates in the professional development of others. Acts as a preceptor and mentor to support the development of nurses, other nurse practitioners, students, and other health care providers.
