Job Overview:

As part of RWJBarnabas Health s Population Health team you will play an instrumental role in providing support to practices in achieving a model for Improving and managing our Employed Medical Group, Joint-Ventures, and Community Partners in various value-based program; for example, Medicare and Commercial Shared Savings Program, Primary Care First Program, etc.

Responsible for monitoring and analyzing relevant issues that will impact RWJBarnabas Health, Accountable Care organization and inform respective team of necessary actions referencing Center for Medicare and Medicaid Services various online portals and tools.

Qualifications:

Required:

  • Associates Degree required
  • Excellent organizational, project management, and relationship management skills required.
  • Must possess, or within 30-days a valid New Jersey Driver license.

Preferred:

  • Bachelors Degree in a healthcare related field preferred or equivalent experience

Essential Functions:

  • Responsible for obtaining documents from regulatory agencies and/or participating providers accurately and timely.
  • Ensure standards of practice and policies are compliant with the ACO contractual requirements and other contractual and regulatory guidelines and standards.
  • Provide day-to-day management of ACO activities, including project management, problem resolution and other activities to assist in the facilitation of a successful relationship with assigned providers.
  • Support roster management including management of individual contractual conditions that may affect the relationship.
  • Act as subject matter expert for ACO and other Governmental Program and ensure availability to support all questions/issues which may arise on a daily basis i,e,, triaging questions internal and external, tracking responses.
  • Participate in ACO CMS Webinars as well as internal/external meetings clinical, data, legal, etc.
  • Responsible for compliance with Public Reporting requirements.
  • Assists in GPRO Quality reporting; manage various vendor s performance associated with the reporting and collaborate with the Care Management team.
  • Provide onboarding to new ACO providers and maintain communication with existing ACO providers.
  • Lead the ACO- Board Meeting, establish and manage the sub-committee meetings as necessary.
  • Meet with CMS Representative monthly.
  • Develop partnerships and collaborations with internal and external service providers to assist in implementing best care practices.
  • Other duties as assigned
