Job Overview:

The Patient Navigator provides short-term therapy services, and connects clients to appropriate levels of care to assist with stabilization. Conducts individual, family and group therapy as needed. Conducts crisis counseling and is knowledgeable of resources to assist in client stabilization. Employs, and is knowledgeable of evidence-based practices for clients with a broad range of mental health and substance abuse issues. Acts as an interdisciplinary team member with ongoing communication with colleagues and doctors. Complies will all state and federal regulations. Employs excellent time management skills. Keeps records and statistics relevant to treatment. Is responsible for reporting identified safety issues in our high reliability organization. Participate in quality improvement, quality assurance and high risk committees as applicable. Is responsible for reporting identified safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors, etc

Qualifications:

Required:

  • Master's Degree in Social Work
  • LSW license
  • Prior experience in Psychotherapy

Preferred:

  • LCSW license

Scheduling Requirements:

  • Day Shift
  • Full-Time
  • 40 hours per week
