James and Sharon Maida Geriatrics Institute Background:

The James and Sharon Maida Geriatrics Institute encompasses an 18- bed ACE (Acute Care for Hospitalized Elders) unit and a comprehensive outpatient geriatrics practice offering both primary care and consultative geriatrics care as well as palliative care consultation and follow up. The Maida Institute includes physical therapy services with an onsite gym, occupational therapy (including training in an ADL (Activities of Daily Life) model kitchen, vestibular rehabilitation, and audiology services. The Maida Institute offers patients and caregivers extensive social work support programs ranging from support groups, counseling, and community referrals, to bereavement services. The Better Health Program at the Maida Institute offers free membership and a variety of social and educational programs that combined with onside health coaching serve to help the surrounding senior community approach the health concerns in aging with camaraderie and knowledge. Monmouth Medical Center Southern Campus's Geriatrics programming also includes a Geriatrics Emergency Medicine Program.

The James and Sharon Maida Institute serves as a community and State-wide resource for geriatrics care, currently employing 2 physicians, a nurse practitioner, one nurse, 2 geriatrics pharmacists, 1 social worker, 2 health coaches, and a program director, as well as 4 physical therapists, 2 occupational therapists, a full time audiologist, and a rehabilitation medicine director. The Ambassador Program will serve to link these comprehensive services and professionals to the older adult communities as well as seniors living in assisted living and nursing home settings.

The James and Sharon Maida Institute at Monmouth Medical Center Southern Campus in Lakewood New Jersey is located in Ocean County. There are approximately 92 senior communities, 20 assisted livings, 35 long term care/nursing homes, and 10 senior day programs in the surrounding local area. The ambassador program was developed from the need for onsite geriatrics expertise with links to Maida Institute professionals at varying schedules and frequency in the variety of care settings previously described. The ambassador program will provide onsite medical care to these programs based on individual need. The program will be managed and directed by a Geriatrics certified Adult Nurse Practitioner. On site clinical care will be provided on site to interested locations, as well as at the institute for more complicated or involved needs. Transportation to patients in the Ambassador Program will be provided by MMCSC's van for needed appointments or follow up testing. The program will utilize the same electronic medical record used at the Maida Institute to allow for review, follow up and after-hours coverage of particular patient issues. Facilities can choose from a range of options for on-site patient care (for example, weekly, bimonthly etc..)

The Maida Institute Ambassador Program will also arrange for on-site health education programs as well as screenings and preventative care (vaccination) events as requested by the facility involved.

Job Summary:

The Advanced Practice Nurse Geriatric Ambassador Program Lead is responsible for performing duties independently following established and standardized procedures and methods referring questionable situations to physician. The APN provides primary health care through assessment, diagnosis, clinical judgment and management to restore, maintain and improve the health status of patients

Essential Functions:

  • Administers injections, immunizations and oral medications, when appropriate
  • Assists physician by recording patient progress notes, issues diagnostic orders, and transcribes orders.
  • Assists with various counseling and education functions.
  • Billing for patient services based on CMS and NJ State Board Regulations.
  • Demonstrates satisfactory knowledge and judgment to provide appropriate care for patients.
  • Develops plan of care for patients in coordination with multidisciplinary team and completes appropriate reports to physicians and family.
  • Documents and maintains patient records of services provided according to facility and nursing standards.
  • Establishes a collaborative relationship with other medical providers.
  • Examines patients to determine presence of illness, utilizing physical findings, laboratory results, medical history, ascertains nature and extent of illness and prescribes treatments as indicated.
  • Interviews, instructs and advises patients regarding health and illness prevention. Instructs patient and family regarding medications and treatment instructions.
  • Issues prescriptions to patients based on Federal & State Regulations.
  • Orders and reviews appropriate laboratory and diagnostic procedures. Reviews all lab data and documents.
  • Participates in the development of protocols.
  • Performs and obtains a completed medical history and physical data on patients with appropriate documentation and findings.
  • Performs independent evaluations and treatment procedures in emergency situations.
  • Provides monitoring and continuity of care between physician visits.
  • Responsible for maintaining the quality and adequacy of nursing practice.
  • Reviews and audits charts routinely.
  • Works in collaboration with a physician to diagnose conditions as necessary.
  • The Geriatric APN will also work with the on-site team at The Maida Institute, to provide coverage when needed as well as patient follow up phone calls, renewals of medication, and education/screening assistance with Better Health community programs.

Job Requirements:

  • Master's degree in Nursing required.
  • Registered Professional Nurse with current NJ state license; CDS and DEA.
  • CPR certification.
  • NP Certification.
  • Three to five years of work experience as a professional nurse.
