Role and Responsibilities:

The Lead Licensed Clinical Social Worker works with high- and rising-risk patients as a behavioral health specialist, care navigator, or both. The Lead LCSW will require knowledge of community resources and training in patient navigation, motivational interviewing, and counseling which allows to uncover and address patient s psychosocial barriers to health.

The Lead LCSW is also responsible for delivering patient care services, including assessment, diagnosis, treatment planning and coordination, monitoring, and evaluation of mental health care to individual patients. Employee will participate in fulfilling RWJ Barnabas mission in meeting the health needs of the community. He or she will function in the role of behavioral health provider within the patient care teams, available to serve all patients receiving primary care in the clinic. This will include, but is not limited to, the assessment and treatment of individuals, families, and groups, as well as crisis intervention and engaging with patients as they present for primary care services. The LCSW will collaborate with other involved providers to achieve the best care for patients and will strive to promote whole person improvement of health outcomes.

Behavioral Health Support:

  • Collaborating with PCP and any other behavioral health specialist on integrated care plan development
  • Provide brief psychotherapy (e.g., 6-12 sessions of cognitive-behavioral treatment)
  • Connect patients with available support services in the health system or community

Care Navigation Support:

  • Identify barriers to patient adherence to care plans, including unmet social needs
  • Provide education and self-management support to achieve patient-centered goals
  • Coordinate care and connect patients with community resources to address non-clinical needs

Supervisory Responsibility:

  • The LCSW will provide clinical supervision to less-experienced social workers, either in individual or group formats.
  • Clinical supervision includes discussing specific cases and offering guidance and advice on how to handle difficult or challenging situations. In addition to providing clinical supervision, other responsibilities might include seeing individual clients, assigning cases, signing off on treatment plans, running staff meetings, performing administrative duties and conducting staff evaluations.

Qualification and Education Requirements:

  • The LCSW by its definition will have: graduated with a Bachelors degree and Master of Social Work degree from an accredited institution of higher learning, passed Certified Social Worker exam, completed supervision requirements in a New Jersey Board of Social Work approved clinical setting and passed the LCSW boards.
  • The LCSW will be responsible for practicing within the scope of their license and maintaining appropriate CEU and licensure requirements.

Preferred Skills:

  • Needs to have excellent communication and management skills.
  • Needs to be able to work well with others and feel comfortable providing direction and guidance to their direct reports.
  • Requires the ability to develop a trusting and supportive relationship with their employees.
  • LCSW supervisors should have good crisis intervention skills and have the ability to remain calm and handle high levels of stress in crisis situations.

These activities are provided telephonically and in a practice or home setting based upon predefined criteria and assignments. This position requires traveling between facilities and locations

Success in this position will lead to improved health for the patient and reduced health care costs for the managed population of patients.
