Provides medical case management services to CCC patients and their families. Conducts a comprehensive initial assessment of the patient and their families which includes demographic information, psychological status, environmental stability, financial stability, and social support as well as an assessment of health education needs.
Outpatient Social Worker (Cancer Center) – Team members in these social work Jobs assist patients and their families in obtaining the maximum benefits of available health care services through psychosocial support with assessment, counseling, transportation, grief counseling, hospice referrals and all other referral services related to breast cancer.
