Case Manager, HIV Services
In this role, the Case Manager becomes part of a compassionate, patient-centered environment dedicated to supporting individuals living with or affected by HIV. Working as a member of an interdisciplinary healthcare team, the Medical Case Manager provides supportive counseling, risk-reduction education, care coordination, referrals, and case management services designed to promote positive health outcomes, treatment adherence, and greater self-sufficiency. This position offers the opportunity to make a meaningful difference in the lives of patients by connecting them with appropriate healthcare, social services, entitlement programs, and community resources while helping address barriers to ongoing care. The Case Manager also plays an important role in maintaining accurate documentation, protecting patient confidentiality, and ensuring that referrals and follow-up services are completed in a timely manner.
A typical day for a Case Manager, HIV Services may include:
Providing medical case management services to patients in accordance with established standards and departmental procedures
Conducting risk-reduction counseling and adherence counseling with patients, with the goal of preventing HIV transmission and promoting positive health outcomes
Providing patients with education, support, and information related to HIV care, treatment adherence, and available community resources
Providing counseling and information regarding advance directives and assisting patients with the completion of appropriate forms
Assessing patient needs and referring and linking patients to healthcare, social services, entitlement programs, and community resources
Helping patients access appropriate services that promote positive health outcomes, treatment adherence, independence, and greater self-sufficiency
Providing supportive counseling and referrals based on individual patient needs and identified barriers to care
Following up on referrals to ensure that patients are successfully connected with recommended services
Monitoring referral activity and completing timely documentation to ensure appropriate continuity and follow-up of patient services
Maintaining accurate, complete, and up-to-date records of all patient encounters and required documentation in the medical record
Responding promptly and professionally to verbal and written requests from patients, interdisciplinary team members, administration, medical agencies, and public agencies
Participating in scheduled medical case management meetings and collaborating with members of the interdisciplinary healthcare team
Functioning effectively as a member of an interdisciplinary team to coordinate patient care and address social, medical, and supportive service needs
Maintaining HIPAA requirements and protecting patient confidentiality when interacting with patients, families, personnel, healthcare providers, and the general public
Communicating respectfully and effectively with patients and families while maintaining appropriate professional boundaries
Promoting continuity of care by coordinating services and communicating relevant information with appropriate members of the care team
Maintaining compliance with unit and departmental scheduling guidelines
Promoting and fostering a positive, respectful, and collaborative work environment
Following departmental policies, procedures, documentation standards, and established case management practices
Performing other duties and responsibilities as assigned in accordance with departmental needs

You are dependable and responsive when addressing verbal and written requests from patients and members of the healthcare team
You value collaboration and enjoy working with professionals from different disciplines to coordinate comprehensive patient care
You approach patients with compassion, respect, patience, and an understanding of their individual circumstances

This position requires a Bachelor's Degree in Social Work or a Social Work-related field. Experience and knowledge of HIV, entitlement programs, and community resources is preferred. Candidates must demonstrate knowledge of case management standards and the ability to function effectively as a member of an interdisciplinary team. Strong communication, organizational, documentation, counseling, and follow-up skills are important for success in this role. The Medical Case Manager must also demonstrate a strong commitment to HIPAA requirements, patient confidentiality, professional communication, and timely coordination of patient services.
