Home Friend Program Coordinator

In this role, the Home Friend Program Coordinator supports the daily operations of the Home Friend program, helping ensure that older adults and individuals with disabilities receive compassionate, dependable, and appropriate in-home support services. Working closely with the program manager, Home Friend staff, volunteers, clients, and community partners, this position plays an important role in coordinating services that promote client safety, independence, dignity, and quality of life. The Home Friend Program Coordinator conducts in-home assessments, coordinates the assignment and dispatch of Home Friend workers, assists with staff support and program administration, and maintains accurate information for required reporting to the Essex County Division of Senior Services. This position also provides direct light chore services when a Home Friend worker is unavailable, helping maintain continuity of care and ensuring that clients' essential needs are addressed. Through effective communication, organization, teamwork, and a client-centered approach, the Home Friend Program Coordinator helps the program operate efficiently while supporting the well-being and independence of the individuals it serves.

A typical day for a Home Friend Program Coordinator may include:

  • Conducting in-home assessments of older adults and individuals with disabilities to evaluate their needs and determine appropriate Home Friend services.
  • Coordinating and dispatching Home Friend workers to clients' homes based on service needs, staff availability, and program requirements.
  • Assisting the program manager with the daily operations and ongoing administration of the Home Friend program.
  • Providing support and assistance to Home Friend staff to help ensure the timely and effective delivery of services.
  • Monitoring service assignments and helping coordinate coverage when staffing changes or scheduling needs arise.
  • Performing light chore services in clients' homes when a Home Friend worker is unavailable, helping ensure continuity of essential support services.
  • Preparing, organizing, and maintaining program data for monthly reports submitted to the Essex County Division of Senior Services.
  • Maintaining accurate and up-to-date records related to client assessments, service coordination, worker assignments, and program activities.
  • Communicating professionally and compassionately with clients, families, Home Friend workers, volunteers, supervisors, and other relevant contacts.
  • Responding promptly to client needs, staff questions, scheduling concerns, and requests for assistance.
  • Supporting the program manager with staff coordination, day-to-day problem-solving, and activities that contribute to effective program operations.
  • Assisting with Volunteer Services as needed, including supporting coordination and other program-related activities.
  • Protecting client confidentiality and ensuring that sensitive communications and records are handled appropriately.
  • Establishing and maintaining respectful, supportive relationships with older adults and individuals with disabilities while recognizing their individual needs and preferences.
  • Prioritizing daily responsibilities, organizing assignments, and working independently while seeking guidance from the program manager when appropriate.
  • Collaborating with colleagues and participating in cooperative problem-solving to improve service delivery and program effectiveness.
  • Following established departmental policies, procedures, documentation requirements, and service standards.
  • Identifying opportunities to improve program coordination, administrative processes, and the overall quality of services provided to clients.
  • Performing other duties and responsibilities as assigned in accordance with departmental and program needs.

  • You work well as part of a team and are willing to assist the program manager with staff coordination and daily operational responsibilities.
  • You can work independently, exercise sound judgment, establish priorities, and seek guidance when appropriate.

This position requires

  • A high school diploma or equivalent 
  • Must have current computer skills, excel, Microsoft 365 etc,

  • Must have a current and valid driver's license

  • At least two years of experience working with older adults and individuals with disabilities.
  • Candidates must demonstrate exceptional interpersonal skills, extensive computer skills, strong organizational abilities, and effective verbal and written communication skills.
  • The ability to conduct in-home assessments, coordinate staff assignments, prepare program reports, maintain accurate records, and work collaboratively with the program manager and Home Friend staff is essential.
  • Candidates must also demonstrate a strong commitment to client confidentiality, customer service, accountability, teamwork, and professional conduct.
  • The successful candidate will be dependable, adaptable, and capable of working with minimal supervision while recognizing when guidance is needed to ensure that client needs and program objectives are met.
