The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

We are seeking a Per Diem Social Worker MSW at Community Medical Center located in Toms River. The Social Worker will provide social work interventions, including supportive counseling, discharge planning, and psychotherapeutic approaches that address psychosocial distress and patients social determinants of health.  

Qualifications:

Required:

  • Master’s degree in social work required. (MSW)
  • LSW or LCSW licensure required.
  • 2 plus years’ experience in social work required.
  • EARC certification is required within 6 months of hire.

Preferred:

  • Experience within home health or post-acute services preferred.

Scheduling Requirements:

  • Per Diem schedule.
  • Monday – Friday: 8am – 430pm (flexible schedule).
  • Every 6th weekend is required.

Essential Functions:

  • Collaboration with the interdisciplinary team, including physicians, clinical nurses, and others as needed.
  • Conduct patient assessments, maintain comprehensive documentation and participate in interdisciplinary rounds.
  • Execute effective discharge planning strategies to optimize inpatient capacity and maintain smooth patient flow.
  • Oversee seamless transitions of care, detailed discharge planning, and optimized patient flow.
  • Evaluate and address health-related social needs, incorporating social determinants of health into standard care planning.
  • Actively participate in initiatives that improve key performance indicators (such as length of stay, throughput) that align with RWJBH System Goals and ensure continuity of care.
