Service Coordinator
Summary:
The Service Coordinator will facilitate the coordination of services for developmentally delayed children who qualify for Early Intervention EI Services. Assist child family in development of the Individualized Family Service Plan and provide supportive counseling and crisis intervention to child family and advocates for children on state and local level.
A typical day as a Service Coordinator may look like:
- Establishes working relationship with children who are developmentally delayed and their families,
- Coordinates the scheduling of assessments in accordance with guidelines established by Early Intervention DOH
- Assessment of children's service needs in the following areas: social and emotional, developmental, medical, cultural, economic, educational, etc
- Development of Eco Map for all eligible children
- Develop and review the Individualized Family Services Plan, IFSP, for all eligible Part C children,
- Ensures all appropriate notification are provided
- Facilitates the coordination of services for Part C eligible children and families through linkage to service providers and EI programs
- Promoting and facilitating communication among service providers working with the child and family
- Advocating for appropriate services
- Supportive counseling for Part C eligible children and families
- Assist families with transition planning to preschool, special education or other services considered appropriate at age three
- Ability to manage electronic files including data entry into EIMS and DOH database
- Facilitate functioning of the Part C Unit in the absence of the Department Director,
- Participates in the development, evaluation and revision of the Departments policies and procedures
- Other duties and or projects as assigned

To be considered for this opportunity, you must have:
- Bachelors degree or higher from an accredited institution in a related health, human service, psychology, or education field
- At least one year of documented professional experience working with children and families in a counseling, supporting, or case management role
- Documented professional experience is preferably with individuals birth to five years of age and their families
