The Case Manager Registered Nurse develops safe discharge plans in order to reduce length of stay and improve throughout. The CM arranges for durable medical equipment, home IV therapy as well as enteral feeds. The CM additionally refers to post-acute care facilities at the appropriate level of care.

Qualifications:

Required:

  • Baccalaureate degree in Nursing from an accredited school or university, or equivalent RN degree with commensurate clinical experience
  • Minimum of 2-3 years experience in a hospital setting
  • Minimum of one year experience in Case Management/Utilization Management or Home Care
  • Successful completion of all Orientation Programs required by CBMC

Certifications and Licenses Required:

  • Current Registered Nurse License in the state of New Jersey
  • Basic Life Support (BLS) from the American Heart Association

Scheduling Requirements:

  • Full-time
  • Day shift
  • 37.5 hrs weekly
  • 8AM - 4PM, Mon - Fri
  • Weekend commitment as needed
  • Holidays as needed

Essential Functions:
The Case Manager coordinates the healthcare needs of patients in the Hospital by collaborating with members of the interdisciplinary Patient Care Team and the Primary Care Physician on the in-hospital and post discharge plan of care.

Coordinates the care and services of the Cooperman Barnabas Medical Center patient population across the continuum of illness. Assumes responsibility for utilization management and monitoring process. Conducts admission and continued stay review on groups of patients, in accordance with the PRO, special negotiated PPO and/or HMO contracts, and on other classifications of patients as designated by the Utilization/Case Management Review Plan. Coordinates patient care, in conjunction with the medical, nursing and other ancillary staff members. Identifies trends and patterns of resource use and patient care, and makes recommendations in compliance with established policy and procedure.

Ensures continuity of care for those patients with post-hospital health care needs and maximizes the patient s and family s abilities to meet the continuing care needs within their social and financial framework. Uses an understanding of principles of growth and development to assess each patient s age specific needs and provide age specific treatment and care.
