Job Overview:

Under the direction of the Assistant Director of Clinical Pharmacy Services, the Transitions of Care Clinical Pharmacy Specialist will be responsible for reviewing and optimizing medication therapy, in collaboration with other healthcare practitioners, for patients transitioning between acute care and outpatient settings with a focus on patients with Congestive Heart Failure.

The clinical specialist will actively promote medication safety through the identification and analysis of adverse drug events and medication incidents; develop and implement evidence-based and/or cost-savings initiatives; and provide educational expertise and support to patients/families and other healthcare disciplines within the hospital,

Qualifications:

Required:

  • Qualified candidates must have a Doctor of Pharmacy degree from an ACPE-accredited school of pharmacy
  • Completion of a PGY1 Pharmacy Residency in general practice
  • Candidates should also be self-motivated, have good communication, organizational, and interpersonal skills, and possess or be eligible for licensure in New Jersey State.

Preferred:

  • Completion of PGY2 Pharmacy Residency training in Internal Medicine Pharmacy, Pharmacotherapy, or Ambulatory Care Pharmacy

Certifications and Licenses Required (if applicable):

  • Board Certification in Pharmacotherapy and/or Ambulatory Care Pharmacy
  • Experience in Transitions of Care pharmacy practice

Essential Functions:

  • Providing pharmaceutical care to patients with a diagnosis of congestive heart failure by assessing pharmacotherapy needs for both the inpatient and outpatient settings.
  • Performing medication reconciliation, and providing education to patients transitioning to home.
  • Make recommendations to providers that will facilitate optimizing the patient's medication therapy.
  • Evaluate patients' medication adherence and compliance post-discharge by evaluating patient's access to and understanding of their medications for congestive heart failure.
  • Serve as a pharmacotherapy resource for licensed independent practitioners, nurses, and other healthcare disciplines.
  • Work collaboratively and communicate timely and effectively with the Transitions of Care nurse practitioner and other members of the health care team.
  • Participate in performance improvement and quality initiatives designed to improve clinical outcomes in congestive heart failure patients.
  • Support the PGY1 Pharmacy Residency Program and other department/hospital initiatives as needed.
