Job Overview:

The Outreach Community Health Worker (CHW) will primarily maintain a presence in the community with members of the community to promote, motivate and improve healthy living. The Outreach CHW will assist community members in utilizing resources and accessing services to overcome barriers to meet their health goals.

Qualifications:

Required:

  • High School Diploma or equivalent

Preferred:

  • Candidates who are bilingual in Spanish is strongly preferred

Scheduling Requirements:

  • Full-Time; 9A-5P, M-F
  • This position is grant funded through 2025 year end with a strong possibility of future extensions.

Essential Functions:

  • Make direct contact with members in the community to address barriers to access, and the importance of preventive and comprehensive health care
  • Travel to various locations to meet the community where they re at, such as members homes, Faith Based Organizations, Community Based Organizations, homeless shelters, hospital outpatient departments etc.
  • Maintain strong relationships with these organizations
  • Conduct needs assessment using the Social Determinants of Health Tool to ensure community members have no barriers accessing primary care services
  • Help community members with accessing resources for social issues like homelessness, rental assistance, utility assistance, food, transportation, childcare services, substance abuse and other social issues that are impacting their lives
  • Schedule appointments for the members with primary care providers/specialists
  • Identify community members insurance eligibility
  • Assist community members with renewing their insurance online and/or inform the community member that the State also sends out renewal package over mail that need to be completed and sent over
  • Enroll the community members to the CHW program for 30 days and conduct weekly follow-up calls and/or home visits
  • Ensure the community members keep their appointments and get their social needs addressed
  • Provide community members with education for self-management of conditions and help with setting health goals such as planning healthy meals or finding time to exercise
