Job Overview:

The Family Advocate will provide a trauma-informed and person- and family-centered approach to parent education, screening and assessment, resource navigation, and supportive follow-up for families impacted by adverse childhood experiences. The Family Advocate will be integrated into the Strengthening Families Program to build trusted relationships with families and assist with the identification of resources to address and/or prevent negative impacts on the social care needs of the family.

Qualifications:

Required:

  • Bachelor's degree in early child development, human services, sociology, social work, substance use or related field
  • Minimum two years of experience in field of service similar to case management, human service, social work, early child development, or substance use disorders
  • Experience working with families
  • Must provide your own transportation
  • Intermediate level of proficiency in MS Outlook and MS Word

Preferred:

  • Master s degree
  • Trained Strengthening Families Facilitator
  • Ability to read, write, and speak fluently in Spanish
  • Experience with Epic or other electronic health record
  • Familiar with adverse childhood experiences, social determinants of health, trauma-informed care, and social care resource networks

Essential Functions:

  • Administers
  • Provides ongoing follow-up for families
  • Collects and maintains accurate family data using Epic
  • Develops care plans and establishes goals and objectives with families as appropriate
  • Actively works to advocate for families and navigate them to the resources identified
  • Participates in and contributes to agency and community networking/referral systems
  • Assists with enrollment and navigation to all appropriate family resources using a trauma-informed and person- and family-centered approach
  • Participates in the implementation of multiple rounds of the Strengthening Families Program to build trusted and effective relationships with families
  • Conducts comprehensive screenings and needs assessments in order to determine families and individuals' social care needs specific to adverse childhood experiences and social determinants of health
  • Educates families regarding the presence and impact of adverse childhood experiences and social determinants of health assessment tools to facilitate effective service delivery in accordance with family needs and program and research design specifications
  • Maintains contact with providers and community-based organizations to ensure effective referrals to ensure family needs are met in a successful manner
  • Coordinates and documents all services provided using the Epic Community Resource Directory and Healthy Planet Care Plans
  • Performs other duties as assigned
