The Case Manager collaborates with physicians and the multidisciplinary team to ensure patients receive timely care that meets evidence-based practice standards and regulatory and payer requirements. The role integrates utilization management, care coordination, and discharge planning functions to assist patients in achieving positive clinical outcomes and safe transitions to the community. The Case Manager provides patient/family support and education and maintains strong relationships with community providers.
