Job Overview:

Coordinates the Home Infusion for patients needing to go home on IV medication from the hospital setting. Participates in program development, quality assurance and the delivery of care. Remains familiar with all pumps, access devices, drugs and IV access. Develops and maintains relationships with referral services and client base. Acts as a resource for case management.

Qualifications:

Required:

  • High school diploma or equivalent.

Preferred:

  • Associates degree.
  • Previous home care experience is desired.
  • Insurance knowledge is a plus.

Certifications and Licenses Required:

  • NJ Pharmacy Tech license, required.

Scheduling Requirements:

  • 8:30A-4:30P each day
  • Full-time, Day
  • This role will support both Jersey City Medical Center (JCMC) and Clara Maass Medical Center (CMMC)

Essential Functions:

  • Understands and adheres to the company policy and requirements. EG: Snow day and sick time.
  • Understands and participates on different committees or plans. EG Emergency Operation.
  • Willing to be cross trained to work or assist in other areas. Helps co-workers when appropriate.
  • Willingness and flexibility to alternating work schedules on short notice. Will support management and do what is asked.
  • Receives and evaluates referrals for home care. Communicate all information to intake department.
  • Provides education about the programs and services we offer to referral sources and patients
  • Assisting with coordination of care between the referral source, the pharmacy, the nursing department and patient/caregiver
  • Helping Qualitas identify appropriate candidates for home infusion.
  • Positive and effective representation of Qualitas Pharmacy Services and introducing the full scope of services offered to hospital case managers, physicians, provider staff, nursing agencies, other referral sources, and patients
  • Develops, maintains and grows home infusion admissions through positive referral source relationships and responsive, caring customer service

  • Maintains up-to-date referral source records, as well as any other requested records in accordance with Qualitas Pharmacy Services policies, procedures &/or business goals.
  • Discuss insurance benefits and co-pay information with the patient or responsible party
  • Represents Qualitas Pharmacy Services in a professional and positive manner.
  • Communicate all problems with your supervisor.
  • Increase referrals from hospital by 10% for the year.
  • Work Environment and Hazards: Hazards include risk of exposure to communicable disease. Office environment at site, travel will be necessary.
  • Work Contact Group: All staff at site; physicians, DME suppliers, case managers, patients and their families, social workers, and floor nurses.
