Job Overview:

  • To optimize clinical outcomes of patients and promote appropriate antimicrobial utilization, while minimizing unintended consequences of antibiotic therapy and reducing costs, by using prospective monitoring, prescriber feedback, and guideline implementation, in addition to other tools,
  • The pharmacist is responsible for the patient population from newborns to geriatrics,
  • The pharmacist is responsible for reporting identified safety issues such as hazardous environments, adverse drug reactions, medication and medical errors, and daily activities, etc,
  • The stewardship pharmacist exercises considerable independent decision-making,

Qualifications:

Required:

  • BA or BS in pharmacy
  • One year pharmacy experience, preferably in hospital, with residency or comparable experience required
  • NJ licensure

Preferred:

  • Specialized training in infectious diseases
  • Previous hospital experience

Scheduling Requirements:

  • Day Shift
  • 40 hours per week

Essential Functions:

  • Assists with facilitation of patient discharge when applicable
  • Counsels patients on recommendations of the antibiotic team to ensure safe transitions
  • Decreases inappropriate antimicrobial agent use housewide
  • Develop and implement guidelines for antimicrobial agent usage and infection/disease treatment
  • Discusses antimicrobial therapy with Infectious Disease physicians and/or prescribers daily (prospective audit and feedback)
  • Document all identified adverse drug reactions
  • Document activities daily
  • Identifies, reports, and resolves medication safety issues related to antimicrobial therapy
  • Initiate and/or participate in research or quality improvement data collection and presentation. (At least 2 drug use evaluations yearly
  • Meets with Antimicrobial Stewarship Physician daily or as directed to discuss cases and recommendations
  • Monitors antimicrobial therapy
  • Optimize clinical outcomes of patient while minimizing unintended consequences of antimicrobial therapy (i.e., toxicity, superinfections, emergence of resistance, etc)
  • Facilitate and/or create programs for optimizing antimicrobial agent usage (i.e., IV to PO program, streamlining, de-escalation, formulary restriction, etc)
  • Provide education and/or outreach activity (at least 3 per year). This may include medical residents, students, physicians, pharmacy residents, laypersons, etc
  • Provide financial forecasting to the Director of Pharmacy or Administration on new programs, services or antimicrobials, related to infection
  • Provides drug information in a timely and accurate fashion
  • Provides a monthly report of activities to Director or assistant director of pharmacy as assigned
  • Reduce hospital costs related to antimicrobial agents and infection-related length of stay
  • Review and update antibiogram(s) yearly
  • Serve as a preceptor and/or research mentor to pharmacy students or residents as directed
  • Works with Infection Control to limit emergence and transition of resistant organisms
  • Performs all other duties as assigned
