Job Summary

Facilitates the coordination of services for developmentally delayed children who qualify for Early Intervention. Assists child|family in development of the Individualized Family Service Plan. Provides supportive counseling and crisis intervention to child|family. Advocates for children on state and local level.

Essential Job Functions
  • Establishes working relationship with children who are developmentally delayed and their families.
  • Coordinates the scheduling of assessments in accordance with guidelines established by Early Intervention/DOH.
  • Assessment of children's service needs in the following areas: social|emotional. developmental, familial medical, cultural, economic, educational, etc.
  • Development and review of the Individualized Family Services Plan (IFSP) for all eligible Part C children. Ensures all appropriate notification is provided.
  • Facilitates the coordination of services for Part C eligible children|families through linkage to service providers and EI programs.
  • Promoting and facilitating communication among service providers working with the child|family. Advocating for appropriate services.
  • Supportive counseling and crisis intervention for Part C eligible children|families.
  • Assist families with transition planning to preschool, special education or other services considered appropriate at age three.
  • Ability to manage electronic files including data entry into EIMS/DOH database.
  • Facilitate functioning of the Part C Unit in the absence of the Unit Coordinator. Participates in the development, evaluation and revision of the Unit s policies and procedures.
Formal Education and Job-Related Experience
  • One or more of the following acceptable:
  • BA Psychology OR Teacher of the Handicapped OR Social Worker OR BSN.
  • Master's degree in social work (LSW) from an accredited program and one year experience in public health or with children who are chronically ill or disabled.
  • A NJ licensed nurse with a bachelors degree in Nursing (BSN) or Bachelor's degree in Social Work (BSW).


Certification
  • As dictated by profession (i.e. if Teacher of the Handicapped, then appropriate Certification required).
  • Bachelor of Social Work (BSW Certification).
License

As dictated by profession (i.e. if employee is a Social Worker, then valid License required).

Job Specific Contacts

Continual contact with children and their families. Frequent contact and meetings with the Regional Early Intervention Collaborative - Family Link of Union and Essex Counties. Occasional contact with NJ Special Child Health Services. Quarterly contact with Case Managers and Service Coordinators throughout the state. Frequent contact with professionals form resource/referral network and EI Programs.

Physical Demands
  • Typical office environment, may travel by car throughout work day to attend meetings and conduct home visits.
  • Ability to work remotely.

Ability to work independently with a demanding client caseload. Ability to provide supportive counseling and crisis intervention. Exhibits empathy and cultural/ethnic sensitivity to a diverse client population. Effective problem solving abilities.
