Provide treatment, social work services and complex discharge planning needs on Adult Psychiatric Inpatient Unit
Qualifications:
Required:
- Master s Degree in Social Work
- Excellent verbal and written communication skills
- Capable of working on multiple tasks simultaneously
- Expert critical thinking and problem-solving skills
- Self-motivation
- Can break down barriers
- Good organization and time management skills
Certifications and Licenses Required:
- NJ Licensed Social Worker
Scheduling Requirements:
Essential Functions:
- Conduct biopsychosocial for all patients, engage families and/or community support services to assess patients' psychiatric, social, environmental and financial status and meet identified needs
- Develop treatment plans with patients and the interdisciplinary team
- Participate in interdisciplinary treatment planning rounds to assess discharge planning needs, treatment modifications, family involvement, and communicate ongoing interventions
- Educate and engage patients and/or families about treatment, discharge planning, patient needs, as well as availability and eligibility criteria for programs/services benefits to meet those needs
- Creatively negotiate, coordinate, and advocate community-based services to assure the highest level of needed supports, continuation of care and wrap around services
- Document in the Medical Record all assessments, reassessments and interventions to enhance multidisciplinary communications, as well as, to meet all regulatory and reimbursement standards
