Job Overview:

The Advanced Heart Failure Nurse - RN plays a central role in the outpatient management of patients with complex cardiac conditions, including advanced heart failure, cardiomyopathies, and hypertrophic cardiomyopathy - HCM,

Working closely with cardiologists, APPs, and the multidisciplinary heart failure team, the RN provides comprehensive clinical support, patient education, medication management, and ongoing care coordination to optimize patient outcomes and quality of life,

This role is ideal for an RN who is looking for a fast-paced, highly specialized environment and enjoys building strong long-term therapeutic relationships with patients and families.

Qualifications:

Required:

  • BSN
  • Graduate of an accredited school of nursing required
  • A minimum of 3 years’ experience, with at least 1-2 years of those in cardiology and/or heart failure patients
  • Ability and interest to learn clinical information and criteria
  • Strong computer literacy and communication skills, such as Microsoft Excel, Word, Teams, and Power Point

Preferred:

  • Knowledge of standard of care treatment for heart failure
  • Strong cardiac background
  • Heart Transplant experience
  • Critical Care experience

Certifications and Licenses Required:

  • Active NJ RN license or compact RN license with NJ endorsement
  • BLS from the American Heart Association
  • ACLS from the American Heart Association

Scheduling Requirements:

  • Day Shift,
  • Full-Time, 37.5 hours
  • Monday – Friday, position requires on call /and weekend and holiday hours

Essential Functions:

  • Participates in the development and implementation of health promotion programs for patients and their families
  • Designs health maintenance and disease prevention interventions for patients
  • Provides anticipatory guidance and counseling of patients/families and assists them to cope with competing concerns of multi-generational family members
  • Evaluates patient s and caregiver s support systems
  • Facilitates patient s transition between health care settings, such as hospital to home or to a skilled nursing facility
  • Manages commonly occurring acute and chronic physical and mental health issues

Patient Management: Heart Failure

  • Identifies that patient assigned is appropriate for Heart Failure team management
  • Appropriately identifies diastolic and systolic heart failure and their management
  • Appropriately identifies different cardiomyopathies
  • Correctly interprets results of diagnostic studies
  • Accurately interprets hemodynamics from right heart catheterization or indwelling pulmonary artery catheter
  • Ensure accurate medication reconciliation upon admission
  • Provides individualized HF education to the patient and his/her caregivers
  • Activate Heart Failure pathway
  • Provides appropriate referrals
  • Validates that patient is receiving guideline directed medical therapy
  • Appropriate collaboration with attending physician, consulting cardiologist, or other specialties as needed
  • Ensure accurate medication reconciliation upon discharge

Other Duties:

  • The Heart Failure Transplant Coordinator, will collaborate with the clinical care team in the ambulatory setting managing patients with advanced heart failure
  • Conduct office based patient assessments, including symptom review, vital signs, medication history, and evaluation of disease progression
  • Monitor and follow patients ensuring timely follow up and escalation of care in collaboration with the Advanced HF team3,
  • Triage patient calls, mychart messages, remote devices, symptom reports, using clinical judgment to determine urgency and appropriate interventions or care coordination
  • Support management of patients on Guideline Directed Medical Therapy GDMT including titration protocols per plan of care
  • Monitor lab results, imaging, and diagnostic testing, communicate findings to providers, and patients as needed
  • Coordinate care and referrals including facilitating prior authorizations, medication programs, specialty pharmacy, testing, evaluations, workups for advanced therapies, and ensuring continuity of care
  • Provide individualized patient and caregiver and family education on disease progression and processes, lifestyle modifications, medication adherence and plan, symptom monitoring, activity recommendations, genetic implications of disease
  • Maintain accurate, timely documentation in the patient record ensuring compliance with clinical protocols, regulatory standards, and team quality metrics
  • Participates in departmental quality and performance improvement initiatives, including data collection and program development
