The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

Our case managers structure their schedules to have the flexibility to accommodate the schedules of the families that we service. To ensure coordination case management of services for children, young adults, and their families who are referred to Special Child Health Services with appropriate private, local, state, and federal agencies that will meet their needs. Case managers strive to maximize access to core system of care components by linking families to services that ensure the well being of the children as well as the family. The mission of Special Child Health Services is to provide case management services to the children of Hudson County from birth through age 21 who have medical diagnoses and or are diagnosed with a developmental disability, and to their families.

Qualifications:

Required:

  • Master’s degree in Social work or Nursing
  • NJ licensed Registered Nurse (RN) OR NJ licensed Social Worker
  • Valid Driver’s License
  • 2 years of experience in public health and/or pediatrics hospital, clinic, etc.

Certifications and Licenses Required:

  • NJ licensed Registered Nurse (RN) OR NJ licensed Social Worker
  • Valid Driver’s License

Scheduling Requirements:

  • Full Time
  • 40 hours/week
  • Office operating hours between 7:30am and 6:00pm

Essential Functions:

  • Partner in decision making at all levels and will be satisfied with the services that they receive
  • Provide family centered, coordinated, ongoing, comprehensive care within a medical home
  • Adequate private and or public insurance and financing to pay for the services they need
  • Children are screened early and continuously for special healthcare needs
  • Community based services are organized so families can access and use them easily
  • Youth receive transition to adulthood services necessary to make transitions to all aspects of adult life, including adult healthcare, work, and independence
  • All families will receive care that is culturally competent, and linguistically appropriate
  • Must have the use of personal vehicle and be available to travel, as needed
