The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.
The Pharmacist fulfills daily operational responsibilities which assure the safe and effective use of medications in our patients,
The pharmacist adheres to contemporary standards of practice in fulfilling these responsibilities,
Education
Doctor of Pharmacy from an ACPE accredited school required,
Current NJ Pharmacy License required,
NJ Board Registration required,
Experience
Minimum of one year accredited pharmacy residency strongly preferred,
Previous EPIC EHR experience preferred,
Previous hospital experience strongly preferred,
Previous Oncology experience desired,
Interpersonal relationships are critical,
Must be able to work cooperatively with all hospital and Pharmacy staff,
Job Specific Requirements
Maintains current patient specific information in the prescribed format,
Checks all profile entries for accuracy and completeness,
Reviews physician orders for
Appropriateness of drug, dose, frequency, and route of administration
Therapeutic duplication
Age appropriateness
Allergies or sensitivities
Current or potential impact as evidenced by laboratory values and food drug and drug drug interactions
Compares new orders with current patient specific information and resolves all drug related problems, in conference with prescribing physician when necessary,
Provides pharmacy documentation of interventions,
Communicates and confers with both nursing staff and patients,
In order to obtain relevant clinical information,
To provide information to support the nurses responsibilities for administering medications, monitoring drug therapy, and providing patienteducation,
When appropriate in order to obtain relevant clinical information,
When possible, and when appropriated, educates patients regarding medications prescribed at discharge,
Prioritizes the processing of medication orders according to clinical importance and other relevant factors,
Emergency needs are given top priority and fulfilled as soon as possible,
The pharmacist supervises assigned technical staff by
Prioritizing tasks and overseeing preparation and checking all medications prior to dispensing for accuracy and completeness,
Delegating other appropriate tasks and checking for accuracy,
Communicates frequently with the technician and uses effective supervisory skills,
Prepares medication when necessary or appropriate, including chemotherapy, parenteral nutrition, large volume parenterals, IV piggybacks, and extemporaneous compounding,
Utilizes sterile technique methods for the preparation of sterile products,
Provides solicited and unsolicited drug information and recommendations to prescribing physicians to assure safe, efficacious, and cost-effective drug therapy and documents suggestions,
Recommendations include, but are not limited to
Pharmacokinetic consultation
Parenteral nutrition consultation
Switching from intravenous to oral formulations of medications
Medication renewal
Responds to all Code Blue and Code White alerts,
Prepares all medications used in the resuscitation attempt,
Assists in other ways at the direction of the physician in charge,
