Job Overview
The Community Health Worker is a vital, community-facing team member dedicated to providing compassionate, non-medical care and advocacy for patients within their residences and local community settings. Operating under the NSG Grant-Transitional Care department, this professional conducts home visits and needs assessments using the Social Determinants of Health framework to support recently discharged individuals. The role focuses on helping vulnerable populations overcome social barriers—such as homelessness, substance abuse, and food insecurity—while guiding them toward long-term health goals, medical compliance, and independent living.
Qualifications
Required:
- High School Diploma or General Education Degree (GED) preferred.
- Must possess a reliable vehicle and a valid, unencumbered driver’s license issued in the employee’s state of residence.
- Proven ability to read, write, carry out directions, and maintain clear records.
- Demonstrated maturity, empathy, and a sympathetic attitude toward the care of the sick and elderly.
- Successful completion of required facility orientation and designated non-medical training programs.
- Must meet the physical demands of the role, including heavy lifting (frequently up to 75 lbs, occasionally over 100 lbs), extensive bending, and standing or physical activity for extended periods.
- Comfortable with frequent driving and regular exposure to various outdoor weather conditions.
Preferred:
- Prior experience working directly within underserved urban communities or community outreach programs.
- Familiarity with local Elizabeth/Union County community-based organizations and social support infrastructure.

Essential Functions
- Home Visits & Assessments: Meets recently discharged patients directly in their residences to conduct comprehensive needs assessments using the Social Determinants of Health Tool.
- Community Sourcing & Social Support: Connects and guides patients through complex social issues impacting their lives, including homelessness, substance abuse, hunger, childcare, and housing.
- Medical Care Coordination: Assists patients with organizing personal records, scheduling weekly follow-up phone calls, and securing appointments with Primary Care Physicians or medical specialists.
- Field Travel & Outreach: Travels flexibly to meet members wherever they are located, including local churches, homeless shelters, and private residences.
- Daily Living Guidance: Empowers patients by guiding them through healthy meal planning, finding time to exercise, grooming, dressing, and housekeeping techniques.
- Incidental Transportation: Provides vital travel assistance for clients to ensure attendance at doctor appointments, grocery stores, or community support events.
- Clinical Communication: Observes, records, and reports any critical changes in the client’s physical condition, behavior, or appearance immediately to a supervisor or Case Coordinator.
- Documentation: Accurately documents all services delivered in strict accordance with Trinitas Regional Medical Center policies and procedures.
- Stakeholder Partnerships: Builds and maintains strong, collaborative relationships with partnering Community-Based Organizations (CBOs).
- Companionship & Engagement: Delivers active companionship and assists in the planning of safe recreational activities and events for isolated clients.
