JOB SUMMARY:
The Community-Based Nurse, Lifestyle Medicine & Clinical Research is responsible for promoting health, preventing chronic disease, and improving health outcomes through evidence-based lifestyle interventions, community outreach, and clinical research activities. This unique role combines community health nursing, lifestyle medicine, and research coordination to address population health needs and support health equity initiatives.
Working collaboratively with the Program Director and interdisciplinary stakeholders, the Community-Based Nurse conducts community health assessments, develops and implements wellness programs, and coordinates clinical research and quality improvement projects. The ideal candidate is passionate about culturally competent care, community engagement, and empowering individuals and families to achieve sustainable health behavior changes.
QUALIFICATIONS:
- Graduate of an accredited School of Nursing required
- Bachelor of Science in Nursing (BSN) preferred
- Current Registered Nurse (RN) license in the State of New Jersey required
- Current and valid driver's license, vehicle registration, inspection, and appropriate automobile insurance coverage required
- Minimum two (2) years of experience in community health, public health nursing, or a related healthcare field required
- Minimum two (2) years of experience in clinical research, quality improvement, or a related healthcare setting required
- Certification or formal training in Lifestyle Medicine preferred, such as through the American College of Lifestyle Medicine
- Completion of Collaborative Institutional Training Initiative Clinical Research Coordinator training or other applicable CITI certification preferred
- Experience working with diverse, underserved, and vulnerable populations preferred
- Bilingual language skills preferred
- Strong organizational, project management, and multitasking abilities
- Excellent communication, patient education, and motivational interviewing skills
- High attention to detail with the ability to adhere to research protocols and regulatory requirements
- Ability to work independently and collaboratively in a dynamic community setting
ESSENTIAL FUNCTIONS:
Community Health & Lifestyle Medicine
- Conduct comprehensive community health assessments to identify population health needs, barriers to care, and opportunities for intervention
- Design, implement, and evaluate evidence-based lifestyle medicine programs focused on nutrition, physical activity, sleep health, stress management, social connection, and substance use prevention
- Provide individual and group health education sessions in community settings, including schools, faith-based organizations, shelters, and other community venues
- Develop culturally responsive outreach strategies that address social determinants of health and promote health equity
- Empower individuals and families to adopt sustainable lifestyle changes that improve long-term health outcomes
- Serve as a trusted resource and advocate for community members seeking healthcare services and support
Clinical Research & Quality Improvement
- Coordinate day-to-day activities for clinical research studies and quality improvement initiatives in accordance with approved protocols and regulatory requirements
- Recruit, screen, and enroll study participants while ensuring ethical and compliant research practices
- Obtain informed consent and maintain accurate research documentation and study records
- Collect, manage, and report clinical and program data while ensuring data integrity and confidentiality
- Monitor study progress and support compliance with institutional, regulatory, and sponsor requirements
- Assist in evaluating program outcomes and identifying opportunities for quality improvement
Program Development & Evaluation
- Collaborate with the Program Director and interdisciplinary teams to support program planning, implementation, research, and evaluation activities
- Participate in development of innovative community health initiatives designed to improve population health outcomes
- Analyze community and program data to identify trends and opportunities for enhancement
- Contribute to grant, research, and program evaluation efforts as appropriate
Community Engagement & Advocacy
- Build partnerships with community organizations, healthcare providers, and local stakeholders to strengthen access to services and resources
- Serve as a liaison between community members and healthcare systems, helping individuals navigate available services and support programs
- Advocate for policies, programs, and initiatives that promote wellness, health equity, and community well-being
- Participate in community events, outreach activities, and educational programs to increase awareness of health promotion initiatives
Professional Practice & Compliance
- Maintain compliance with nursing standards of practice, research regulations, organizational policies, and applicable state and federal requirements
- Demonstrate professionalism, cultural humility, and ethical conduct in all interactions
- Participate in ongoing professional development and continuing education activities related to community health, lifestyle medicine, and research
WORKING CONDITIONS / ADDITIONAL INFORMATION:
- Frequent travel to community-based sites and partner organizations required
- Flexible work schedule including occasional evenings and weekends to support community outreach activities and events
- Work is performed in a variety of settings including community centers, schools, faith-based organizations, healthcare facilities, and participant homes as appropriate
- Position requires a strong commitment to serving diverse populations in non-traditional healthcare environments
