Job Overview
- Conducts thorough patient assessments, including medical history, physical examination and diagnostic test results
- Develops and implements comprehensive care plans for patients undergoing Heart and Vascular procedures and/or admitted for further evaluation/treatment
- Monitor patients hemodynamic status, ordering, adjusting medications and interventions as needed
- Provide pre and post procedure/ heart failure education and support to patients and their families
- Facilitate discharges for cardiovascular service line patients
- Provide education to staff and community regarding cardiac conditions and procedures
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Work closely with a multidisciplinary team, including cardiologists, other NPs, nurses, and social workers
- May perform other duties as assigned or directed by management
- Perform history and physicals, comprehensive assessments and manage the care of critically ill cardiac patients in the CCU
- Develop a plan of care in collaboration with the CCU multidisciplinary team (i,e, intensivist, attending cardiologist, medical resident, cardiology fellow, and CCU Nurses) to develop and implement evidence-based treatment plans
- Manage acute and chronic cardiac conditions such as acute coronary syndrome, heart failure, cardiac arrhythmias , cardiogenic shock, post-operative cardiac surgery recovery and mechanical circulatory support devices (e,g, IABP, Impella, ECMO etc)
- Order and interpret diagnostic tests, medications, and perform advanced procedures within scope of practice and institutional protocols
- Monitor patient progress and adjust care plans accordingly
- Participate in daily CCU rounds and assist with rapid response and code situations
- Communicate effectively with physicians, patients, families, and other team members, providing education and support throughout the care continuum
- Ensure timely and accurate documentation of clinical encounters, progress notes, orders, and care plans in the electronic medical record (EMR) as well as entry of procedural and time-based evaluation/management codes
- Contribute to unit-based quality improvement initiatives and clinical research as appropriate
- Comply with hospital policies, procedures, and all regulatory requirements,
