Summary of Job Function:
The Transitions of Care (TOC) Stroke Nurse Practitioner is responsible for providing comprehensive inpatient and outpatient care for patients discharged from the Comprehensive Stroke Center with Dx Ischemic Stroke, Transient Ischemic Attack (TIA), Intracerebral Hemorrhage (ICH), Subarachnoid Hemorrhage (SAH), and other cerebrovascular disorders
The TOC Stroke NP serves as a key member of the stroke team, ensuring continuity of care following hospitalization, supporting secondary stroke prevention, reducing readmissions, and improving patient outcomes
The NP collaborates closely with vascular neurologists, inpatient and outpatient stroke teams, nursing, rehabilitation services, case management, primary care providers, outpatient clinic staff, and community resources to ensure seamless transitions across the continuum of care
The TOC Stroke NP will participate in both outpatient and inpatient responsibilities as needed and will maintain competency in acute stroke management to support the Comprehensive Stroke Center s operational and patient care needs
Key Responsibilities
Outpatient Clinical Care:
- Conduct post-discharge follow-up phone calls and visits for patients with stroke, TIA, ICH, SAH, and other cerebrovascular diagnoses
- Perform comprehensive neurological assessments and ongoing management of stroke-related conditions
- Review hospitalization, diagnostic testing, medications, and treatment plans
- Perform medication reconciliation and adjust treatment plans as appropriate
- Reinforce secondary stroke prevention strategies and risk factor modification
- Identify complications, barriers to recovery, and patient care needs
- Coordinate diagnostic testing, specialty referrals, and follow-up care as indicated
- Escalate clinical concerns to the supervising vascular neurologist when appropriate
Care Coordination and Readmission Prevention:
- Facilitate timely transitions from inpatient to outpatient care
- Collaborate with inpatient teams, outpatient providers, primary care physicians, specialists, rehabilitation facilities, home care agencies, and community resourcesIdentify and address barriers to care, medication adherence, and follow-up compliance
- Participate in stroke readmission reduction initiatives and quality improvement projects
Education and Community Outreach:
- Provide education to patients and families regarding stroke recovery, warning signs, medications, and risk factor management
- Reinforce BEFAST stroke recognition and emergency response activation
- Participate in community education programs, stroke awareness events, support groups, health fairs, and outreach initiatives sponsored by the Comprehensive Stroke Center,
Quality Improvement and Program Support:
- Participate in Comprehensive Stroke Center quality initiatives, performance improvement projects, and accreditation activities
- Assist with Excel data collection, outcome monitoring, and stroke quality metrics
- Participate in departmental meetings, conferences, and educational activities
Documentation and Billing:
- Complete accurate and timely documentation in the electronic medical record
- Document and bill encounter in accordance with institutional, regulatory, and payer requirements
Acute Stroke Coverage and Cross-Training:
- Complete training and maintain competency in acute stroke evaluation and management
- Maintain proficiency in stroke assessment, NIHSS scoring, mRS , ENLS and stroke protocols
- Provide support and coverage for acute stroke services, inpatient stroke consultations, and stroke alerts as operational needs require
- Participate in ongoing competency assessments, stroke education, and professional development activities
- Maintain yearly requirement of 8 credits Stroke CME:
Schedule and Coverage Expectations:
- Primarily weekday hours (7:00 AM - 3:00 PM, 9:00 AM - 5:00PM) to support inpatient and outpatient coverage based on departmental needs
- Occasional weekend, holiday, and 12-hour shift coverage may be required
- Participation in acute stroke service coverage as operational needs require
Qualifications:
Required
- Masters degree in Nursing from an accredited Nurse Practitioner program
- Current Advanced Practice Nurse license in the State of New Jersey
- Current Registered Nurse license in the State of New Jersey
- National certification through AANP or ANCC
- Current CDS and DEA registration
- BLS and ACLS certification
- Excellent communication, organizational, and interpersonal skills
- Proficient computer and electronic medical record skills,
Preferred
- Prior experience in stroke, neurology, vascular neurology, neurocritical care, inpatient medicine, or transitions of care
- NIH Stroke Scale (NIHSS) Certification
- Modified Rankin Scale (mRS) Certification
- Emergency Neurological Life Support (ENLS) Certification
- Familiarity with Comprehensive Stroke Center certification standards, stroke quality metrics, and performance improvement initiatives
