Pay Range: $125,000.00 - $195,000.00 per year

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Requirements

• APN License
• RN License
• NP Certification 
• CDS and DEA
• ACLS and BLS
• 2+ years of experience in geriatrics

Shift:

• M-F (8:30a-4:30p)
• 4 clinic and consult days
• 1 day follow up home visits for high risk patients recently discharged from the hospital 

Essential Functions

• Provide comprehensive primary care with a focus on geriatric needs and chronic disease management
• See a manageable caseload (about 8-12 patients per day), allowing time for thorough visits and care coordination
• Work collaboratively with the team to support complex patients and promote healthy aging
• Participate in hospital home visit initiative to help reduce readmissions
