Job Overview:
The Advanced Practice Nurse (APN) in the outpatient heart transplant setting provides advanced, evidence-based care across the transplant continuum, including pre-transplant evaluation and listing, and longitudinal post-transplant management of immunosuppression, rejection surveillance, and transplant-related comorbidities
Practicing within an established collaborative practice agreement, the APN performs comprehensive assessments, formulates and implements management plans, orders and interprets diagnostic studies, prescribes and adjusts medications, and coordinates care alongside transplant cardiologists, surgeons, transplant coordinators, transplant pharmacists, social workers, dietitians, and other interdisciplinary team members
Requirements:
- Current RN and APRN licensure in NJ required
- Master of Science in Nursing (MSN) required; DNP preferred
- Current BLS and ACLS certification required
- 3+ years of nursing experience, with at least 1 to 2 years in cardiology, advanced heart failure, or solid organ transplant preferred; prior advanced practice experience in transplant or advanced heart failure strongly preferred
Responsibilities:
- Conducts comprehensive histories and physical examinations for pre- and post-transplant patients, documenting findings and communicating changes in status
- Assists the transplant RN coordinators in transplant evaluation process, including ordering diagnostic tests and consultations, ensuring completion of required work-up elements, and supporting listing and waitlist management
- Manages immunosuppression therapy in the post-transplant period, including ordering and interpreting drug levels and adjusting regimens within the collaborative practice agreement
- Coordinates rejection surveillance per protocol, including scheduling and follow-up of endomyocardial biopsies, imaging, and laboratory studies, and escalates abnormal findings appropriately
- Recognizes, evaluates, and manages transplant-related complications and comorbidities, including infection, cardiac allograft vasculopathy, renal dysfunction, metabolic disease, and malignancy surveillance
- Orders and adjusts infection prophylaxis and vaccinations consistent with protocols
- Provides patient and family education on immunosuppression adherence, infection prevention, lifestyle modification, and long-term graft health
- Performs medication reconciliation, evaluates adherence, and adjusts therapy or escalates as appropriate
- Collaborates with transplant coordinators, pharmacists, and providers to ensure timely follow-up and adherence to transplant and regulatory protocols
- Supports psychosocial assessment processes by coordinating with social work and behavioral health services
- Participates in quality initiatives, data collection, and program improvement activities related to transplant outcomes and regulatory compliance
- Serves as a clinical resource and preceptor for patients, caregivers, nursing staff, trainees, and referring providers regarding heart transplant management,
