PD Physical Therapist – RWJBarnabas Health
At RWJBarnabas Health, the Physical Therapist plays a vital role in helping patients regain mobility, improve function, and achieve their highest level of independence. Working collaboratively with an interdisciplinary care team, they evaluate patient progress, provide evidence-based therapeutic interventions, educate patients and caregivers, and support safe, effective discharge planning while delivering compassionate, high-quality care.
A typical day for a Physical Therapist may include:
- Evaluates patients and implements individualized treatment plans designed to improve mobility, strength, balance, and functional independence
- Delivers skilled physical therapy interventions consistent with the established plan of care while modifying treatment approaches based on patient response and progress
- Educates patients, families, caregivers, and healthcare team members using effective teaching methods to support rehabilitation goals and ongoing recovery
- Collaborates with physicians, nurses, case managers, and rehabilitation colleagues to coordinate care and optimize patient outcomes
- Assesses changes in patient condition, communicates findings promptly, and recommends plan of care adjustments when appropriate
- Documents patient evaluations, treatments, progress notes, billing, and other required records accurately and in compliance with organizational policies and regulatory standards
- Assists in developing discharge plans by reinforcing patient goals, anticipated outcomes, and recommendations for continued care
- Identifies medical, social, and psychological factors that may impact rehabilitation and communicates concerns to the interdisciplinary team
- Provides guidance to nursing staff and other healthcare professionals on safe patient mobility, positioning, and rehabilitation strategies
- Reviews medical records to obtain relevant clinical information and ensure continuity of care throughout the rehabilitation process
- Reports safety events, near misses, and other incidents in accordance with organizational policies while maintaining a strong commitment to quality and patient safety
- Performs additional departmental responsibilities as needed to support patient care and operational excellence

To be considered for this Physical Therapist opportunity:
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program required
- Current New Jersey Physical Therapist license in good standing required
- Current BLS certification through the American Heart Association required
- Previous experience in an acute care, transitional care, rehabilitation, or hospital setting preferred
- Strong clinical assessment, documentation, communication, and interdisciplinary collaboration skills required
- Ability to provide safe, effective, evidence-based physical therapy services while maintaining compliance with regulatory and organizational standards
