Are you a dedicated social worker ready to provide vital, frontline advocacy for patients across the healthcare continuum? At RWJBarnabas Health, our Case Management teams work collaboratively to support patients and families navigating the social, financial, and psychological impacts of illness. As an essential member of our interdisciplinary teams, you will partner directly with physicians, nurses, and allied health professionals to sensitize providers to the emotional components of care and ensure patient needs are championed. Utilizing your clinical case management skills, you will help redefine the patient experience through compassion, resource navigation, and teamwork. As a Social Worker MSW, a typical day might include the following:
• Identifying, screening, and completing comprehensive clinical assessments for cases across assigned patient populations.
• Formulating and implementing timely, appropriate, and safe discharge plans tailored to each patient’s unique needs.
• Providing supportive counseling to patients and their families regarding illness, coping with hospitalization, and complex care transitions.
• Connecting patients with critical community resources and conducting diligent, structured follow-ups to ensure continuity of care.
• Maintaining precise monthly statistics and entering accurate, clear, and timely documentation of all social work interventions into the patient’s electronic medical chart.
• Participating actively in interdisciplinary team rounds to review caseloads, align care paths, and maintain collaborative working relationships.
• Coordinating social work services within the palliative care team and facilitating patient-family meetings for the purpose of goal setting and advanced care planning.
• Coordinating educational and outreach services relevant to the social work-palliative care collaborative.
• Serving as a vital liaison between the palliative care team and hospital quality improvement, discharge planning colleagues, community hospices, home care agencies, and long-term care facilities.
• Participating in educational in-services, departmental meetings, and external seminars to bring significant new information and resources back to the entire department.
• Providing field instruction and mentorship for graduate or undergraduate social work students, as available.

To be considered for this opportunity, you must have:
• A Master’s degree in Social Work (MSW) from an accredited School of School Work.
• An active Social Work Licensure in the State of New Jersey (LSW or LCSW).
• The flexibility to participate in a weekend and holiday work rotation alongside your departmental team.
• Previous medical social work or acute care hospital experience is strongly preferred.
• Bilingual language skills in English and Spanish are preferred.
