Job Overview:

Qualifications:

Required:

  • Strong communication and organizational skills
  • Proficient computer skills

Certifications and Licenses Required:

  • BLS through American Heart Association upon hire
  • ACLS Certification within 6 months of start date
  • Active New Jersey  Registered Nurse License or active Compact Registered Nurse License with New Jersey endorsement

Preferred:

  • Med./Surg. Telemetry and/or ER nursing experience
  • National nursing certifications

Scheduling Requirements:

  • Full Time 
  • Day Shift
  • 8am-4:30pm
  • Monday – Friday

Essential Functions:

  • The EIP (Early Intervention Program) is a grant-funded program that provides HIV Care & Treatment to patients age 18 and older. Through the use of a multidisciplinary team approach, individualized treatment plans are developed for each patient with the goal of achieving and maintaining an undetectable HIV viral load.
  • The RN, as part of the team, will conduct intakes, assessments, and screenings of the patient and provide education in the area of medication adherence, risk reduction behaviors and nutritional counseling.
  • Qualified candidates will ideally have experience in caring for HIV positive patients, proficiency in venipuncture skills and bilingual Spanish would be a plus.
  • Answers telephone on the third ring
  • Takes accurate/complete messages (name, date, time, telephone number, short detailed message)
  • Prioritizes messages
  • Obtains written permission from the client/guardian to work with another department/agency on their behalf
  • Provides complete information to the requesting department/agency
  • Reviews clients mailing address prior to sending information/ documents to clients
  • Maintains a supply of forms/patient information needed on a daily basis (ADDP, taxi vouchers, chart forms, etc,)
  • Completes documentation within 24 hours of client contact
  • Assures that all necessary signatures are present on the medical record
  • Assures that all clients receive a follow-up appointment, PPD reading, blood work and gynecology examination (if applicable)
  • Documents all telephone orders from the physicians and assures that those orders are signed on the next clinic day of that individual physician
  • Documents the content of all telephone triage conversations from the compliant to the recommendation(s) regarding same
  • Documents all pertinent information regarding the client on the appropriate documentation tool, including but not limited to the following: - Opportunistic infections
  • Documents all pertinent information regarding the client on the appropriate documentation tool, including but not limited to the following: - Vaccines
  • Documents all pertinent information regarding the client on the appropriate documentation tool, including but not limited to the following: - PPD
  • Documents all pertinent information regarding the client on the appropriate documentation tool, including but not limited to the following: - Medications/Refills
  • Documents all pertinent information regarding the client on the appropriate documentation tool, including but not limited to the following: - Hospitalizations
  • Documents all pertinent information regarding the client on the appropriate documentation tool, including but not limited to the following: - Laboratory results
  • Documents all pertinent information regarding the client on the appropriate documentation tool, including but not limited to the following: - Radiology results
  • Documents all pertinent information regarding the client on the appropriate documentation tool, including but not limited to the following: - Special procedure results
  • Provides education in the following areas (not limited to these areas): - HIV/AIDS (definition, progressions of disease, opportunistic infections, etc, )
  • Provides education in the following areas (not limited to these areas): - Risk reduction/behavior modification
  • Provides education in the following areas (not limited to these areas): - Medication administration (injectables)
  • Provides education in the following areas (not limited to these areas): - Medication dosing, side effects, contraindications and basic pharmokinetics
  • Provides education in the following areas (not limited to these areas): - Nutrition
  • Provides education in the following areas (not limited to these areas): - Hygiene
  • Provides education in the following areas (not limited to these areas): - Laboratory results
  • Provides education in the following areas (not limited to these areas): - Need to return to the clinic for follow up physician/nurse/case manager visits
  • Initiates and coordinates with the nursing staff and the physicians an interdisciplinary review of all clients seen in the clinic
  • Submits complete, accurate statistics and other requested information to the Office Manager/Director by the deadline given
