For children and families navigating complex healthcare needs, having the right support at the right time can make all the difference. As a Patient Navigator, you'll help coordinate care for pediatric patients with rehabilitation needs and acute, chronic, or life-altering conditions. You'll serve as a trusted connection between patients, families, medical providers, therapists, insurance companies, and community resources, helping families navigate their care journey across the continuum.
As a Patient Navigator, a typical day might include the following:
- Coordinate care for babies, children, and young adults across the continuum of care, including intake, outpatient, inpatient, and long-term care services.
- Serve as a liaison between patients and families, internal and external providers, therapists, admissions teams, administrators, insurance companies, and community agencies.
- Coordinate communication among patients, families, and service providers to support continuity of care.
- Work as an integral member of the multidisciplinary care team and support family-centered care.
- Provide triage for families seeking services to improve the management and understanding of chronic illnesses and determine appropriate service needs.
- Review and organize intake materials and consult with the Chronic Illness Management team as needed.
- Ensure evaluations and treatments are scheduled for prospective patients.
- Assist with facilitating inpatient admissions by coordinating with internal and external stakeholders and multidisciplinary team members as needed.
- Obtain, manage, and document information related to patient referral pathways and case stages.
- Advocate for the needs of patients and families across medical, social, and financial areas.
- Provide necessary documentation to external agencies as needed.
- Help families understand their healthcare options, available resources, and next steps throughout the care process.
- Perform other duties as assigned by the supervisor and in collaboration with other CSH departments.

- Schedule: Day shift, Monday through Friday 8:00am-4:30pm, evenings/weekends as needed, Full-Time 40 hours per week
To be considered for this role, you must have a Bachelor's Degree and experience in care coordination, along with experience working in pediatrics. Candidates must demonstrate excellent communication and organizational skills and the ability to work effectively as part of a multidisciplinary team. A strong understanding of family-centered care, pediatric medical diagnoses and their implications for patients and families is strongly preferred. The ability to coordinate services across the care continuum, communicate effectively with internal and external stakeholders, advocate for patients and families, and manage and document referral information is essential.
