Are you a licensed clinician passionate about delivering trauma-informed, integrated care to individuals navigating the intersection of mental health and chemical dependency? At RWJBarnabas Health, our Integrated Co-Occurring Grant teams operate at the forefront of behavioral health, delivering specialized medical and therapeutic interventions across complex dual-diagnosis populations.
As a Licensed Social Worker (LSW), you will leverage a strengths-based framework to conduct comprehensive psychosocial assessments, facilitate evidence-based individual and group psychotherapy, and design targeted dual-recovery intervention paths. Operating within an innovative grant-funded infrastructure, you will work fluidly alongside a multidisciplinary medical team to break down barriers to care, mitigate relapse triggers, and optimize long-term clinical outcomes. We view our social workers as critical components of the therapeutic milieu, transforming the patient experience through clinical excellence, teamwork, and profound empathy.
A Typical Day
- Biopsychosocial Intakes: Performing detailed clinical psychosocial assessments, interpreting family dynamics, and reviewing medical/substance use histories to accurately define treatment needs.
- Dual-Recovery Treatment Planning: Designing and implementing individualized treatment plans that proactively identify substance use triggers, establish protective support mechanisms, and incorporate 12-step or holistic recovery participation.
- Evidence-Based Psychotherapy: Conducting high-quality individual, group, and family psychotherapy sessions in strict accordance with established departmental practice guidelines.
- Multidisciplinary Collaboration: Partnering fluidly with physicians, nurses, and external community stakeholders during clinical planning conferences to regularly review, update, and modify integrated discharge plans.
- Clinical Compliance & Tracking: Maintaining legally compliant, precise electronic patient charts. Progress notes must be thoroughly documented within 24 hours of the Date of Service (DOS).
- Utilization Management Assistance: Understanding the specific healthcare payor identification process to ensure all delivered clinical services are accurately authorized.
- Milieu Safety Checkpoints: Performing random Urine Drug Screens (UDS) on patients as clinically indicated to monitor treatment compliance and safety.
- Family & Community Linkage: Involving families or active referral sources in all treatment endeavors unless contraindicated, while collaborating with outside social service agencies to ensure a seamless post-acute continuity of care.
This Role Might Be For You If:
- You Are an Advocate for Recovery: You possess a deep, empathetic passion for creating a welcoming, non-judgmental therapeutic environment for vulnerable populations navigating co-occurring diagnoses.
- You Are Evidence-Driven: You keep your clinical practice sharp by staying current on relevant addiction treatment modalities, behavioral health research, and national professional standards.
- You Value Documentation Excellence: You set a high bar for your charting, taking operational pride in meeting strict compliance deadlines and logging precise, daily milestone outcome data.
- You Believe in Strengths-Based Healing: You understand that long-term recovery is optimized when a clinician meets a patient's complex needs with structured vigilance, deep empathy, and a focus on individual strengths.

- Experience: A minimum of one (1) year of post-graduate clinical social work experience working directly within a substance abuse or addiction treatment setting is required.
