Job Overview:

RWJBarnabas Health is seeking a highly dedicated Lead Case Manager for the Case Management department at Monmouth Medical Center Southern Campus.

Qualifications:

Required:

  • BSN or MSW required
  • Current and valid licensure as a Registered Nurse or Social Worker in the state of New Jersey
  • EARC certification is required within 6 months of hire and will be arranged by RWJBH
  • Minimum 2 years of acute care case management experience
  • Demonstrated expertise in discharge planning, care coordination, and utilization management

Preferred

  • If SW, LCSW preferred
  • Certification in Case Management (ACM or CCM).
  • Experience serving as a preceptor, mentor, or informal leader
  • Experience managing complex cases and escalation processes

Essential Functions:

  • Responsibilities inclusive of RN/SW case management role (see Case Manager, RN, and Case Manager, SW, job descriptions. Includes provision of direct case management services, including assessment, care coordination, discharge planning, and interdisciplinary collaboration.
  • Serve as a resource, mentor, and preceptor for case management staff, supporting clinical decision-making and workflow efficiency.
  • Assist with daily workflow coordination, including prioritization of cases, assignment support, and throughput optimization.
  • Identify and address discharge barriers, providing real-time problem-solving and escalating complex issues to physicians, leadership, or hospital administration as needed.
  • Act as a point of contact for operational and clinical escalations during assigned shifts, particularly in the absence of case management leadership.
  • Represent case management in meetings, interdisciplinary rounds, and site- or system-level initiatives as needed.
  • Participate in the development and maintenance of department resources, workflows, policies, and procedures to support standardization.
  • Support onboarding, orientation, and competency development of new staff and team members.
  • Promote compliance with regulatory requirements, including CMS Conditions of Participation, payer guidelines, and documentation standards.
  • Participate in performance improvement and quality initiatives focused on LOS, throughput, patient flow, and discharge outcomes.
  • Demonstrate adaptability and support efforts to streamline processes and improve operational efficiency.
  • Perform other duties as assigned or directed by management.
