Case Manager I – RWJBarnabas Health

At RWJBarnabas Health, our Case Management teams work collaboratively to provide high-quality, compassionate care coordination and comprehensive treatment planning when our communities need it most. The Case Manager I works within a multidisciplinary team of therapists and medical providers to facilitate holistic treatment coordination and care continuity. This role serves as a primary link between patients with severe or persistent mental illness and vital community resources. Key responsibilities include patient advocacy, crisis intervention, professional development, and maintaining environmental safety and regulatory compliance. The Case Manager I will take an active role in treatment team meetings to manage coordination of care. We view our Case Managers as a crucial component of our care plan, redefining patient experience through teamwork, empathy, and excellence.

As a Case Manager, a typical day might include the following:

• Manage smooth patient transitions and communication loops between PESS, inpatient units, and outpatient clinics.
• Deliver targeted resource navigation and prepare or present clear, accessible psychoeducational and mental health materials to patients immediately before and after inpatient discharge.
• Facilitate direct referrals to secure external housing, social services, addiction treatment, and physical health providers while working independently to link patients with needed services.
• Facilitate structured, skill-based groups for mental health patients, focusing closely on community resources and transition planning.
• Author accurate, thorough medical records and behavioral health documentation that strictly meet internal policy guidelines, regulatory standards, and departmental submission deadlines to ensure environmental safety.
• Prepare for and actively contribute to interdisciplinary treatment team meetings to optimize patient outcomes and manage coordination of care.
• Attend specialized in-service education, participate in mandatory ongoing training, and actively help plan or prepare professional development and training content when required by leadership.
• Provide active crisis de-escalation and intervention in direct cooperation with other treatment team members.
• Execute varying professional tasks and operational duties as assigned or directed by the department manager.

To be considered for this opportunity, you must have:

• Bachelor's degree in Social Work, Psychology, Counseling, or a closely related human services field required.
• 1–2 years of experience working with individuals with severe or persistent mental health and behavioral health issues preferred.
• Proven experience in crisis de-escalation, maintaining professional boundaries, managing behavioral health documentation, and multidisciplinary communication.
• Knowledge of local community resources and prior Group Facilitation experience running therapeutic or structured skill-based groups are highly preferred.
• Scheduling Requirements: Full-Time, Day Shift (8:00 AM – 4:30 PM, Monday – Friday).
