Job Summary:

Coordinates Home Infusion for patients needing to go home on IV medication from our system hospitals,

Participates in program development, quality assurance and delivery of care,

Remains familiar with pumps, access devices, drugs and IV access,

Develops and maintains relationships with referrals services and client base,

Assists our clinical team by providing documents and information necessary for discharge,

Acts as a resource to case management,

Education Required: Associate degree Preferred or expertise as a pharmacy technician

Additional Education Preferred: Previous Home Care Experience Desired

Insurance Knowledge a plus

Licensure Required: Pharmacy Technician Registration

Additional Licensure Preferred: Pharmacy Technician Certification

Experience Required: none, willing to train the right candidate

Experience Preferred: Home Infusion experience preferred

Job Responsibilities (list 8-12):

1,

Assist with coordination of care between the referral source, the pharmacy, the nurse department and patient/caregiver,

2,

Develops, maintains, and grows home infusion admissions through positive referral source relationships and responsive care and customer service,

3,

Discuss insurance benefits and co-pay information with the patient or responsible party

4,

Helps RWJBH ISP identify appropriate candidates for home infusion

5,

Increase referrals from hospital by 10% for the year

6,

Maintains up-to-date referral source records, as well as any other required records in accordance with RWJBH Infusion and Specialty Pharmacy Services policies, procedures and or business goals,

7,

Represents RWJ Infusion & Specialty Pharmacy in a professional and positive manner

8,

Provides education to patients on what to expect about their care at home

9,

Provides education about the programs and services we offer to the referral source and patients

10,

May perform other duties as assigned or directed by management

Demonstrates professionalism with patients and their caregivers, referral sources, co-workers and supervisor

Ability to perform test claims in Care Tend for patients with prescription drug plans

Ability to check pricing and availability of medications in Care Tend for patients

6,

Ability to direct and assist physicians and case managers on how to submit a prescription

7,

Ability to solve problems independently and if needed ask for help from my direct report
