The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Summary:

The Community Health Educator supports community health strategy by coordinating, implementing, monitoring and evaluating community health programs, screenings and iniatitives including the Better Health program,

A typical day as a Community Health Educator may look like:

  • Works collaboratively with hospital leaders, clinical departments, community organizations, public health agencies, physicians, community partners, and other stakeholders to address social drivers of health and improve access to health and wellness resources,
  • Facilitate development of goals objectives, strategies, action plans, and performance measures
  • Serve as primary hospital liaison to community health coalitions and collaborative initiatives
  • Develop partnerships that advance priorities identified in CHNA
  • Identify health disparities, social determinants of health, emerging community needs and populations experiencing inequities
  • Act as the liaison between hospital and GMPHP to align CHIP and SIP processes
  • Work as an active participant in the community health and Better Health programming to ensure that programming impacts metrics designed to support CHIP & SIP goals
  • Assists with the workgroups in the community implementing plans for hospital and corporate RWJBH to develop and implement, track and measure, CHIP and SIP community health improvement plans,
  • Works with executive lead to develop and manage the hospitals comprehensive CHNA process and timeline,
  • Collect, analyze and interpret quantitative and qualitative community health data
  • Facilitate development of goals objectives, strategies, action plans, and performance measures
  • Serve as primary hospital liaison to community health coalitions and collaborative initiatives
  • Work as an active participant in the community health and Better Health programming to ensure that programming impacts metrics designed to support CHIP & SIP goals
  • Demonstrates the ability to teach and effectively communicate both verbally and in writing
  • Develops educational programs in alignment with medical advisory panel physicians and community health needs
  • Develops and provides educational programs based on the most current information available and in keeping with recommended best practices
  • Establishes rapport with clients
  • Maintains records related to education, screenings, Better Health Member initiatives, etc and provides data reports monthly or as requested
  • Collaboratively develops outcomes measures with time frames and targets
  • Works cooperatively with other staff to achieve the goals of the department,
  • Acts an effective and enthusiastic team member for geriatric initiatives,
  • Presents educational programs in a clear, entertaining and understandable matter

To be considered for this opportunity, you must have:

  • Minimum Bachelors degree in Public Health or related healthcare field
