At RWJBarnabas Health, our Ambulatory Care Registered Nurses help patients and families navigate complex healthcare needs with clinical expertise, compassion, and timely support. As an Ambulatory Care RN in our Hamilton Ambulatory Clinic, you'll provide professional nursing care through telephonic triage, clinical assessment, care coordination, and direct support to ambulatory health programs. Using evidence-based protocols and sound nursing judgment, you'll help determine the appropriate level of care, connect families with needed services, and support safe, coordinated outcomes for children with a wide variety of complex medical needs.

As an Ambulatory Care Registered Nurse (RN), a typical day may include:

  • Providing telephonic triage using established clinical protocols, nursing scope of practice, and organizational guidelines

  • Assessing patient symptoms, severity, and urgency using standardized triage guidelines and professional nursing judgment

  • Providing appropriate clinical guidance, patient and family education, and nursing recommendations within scope of practice

  • Recognizing urgent or emergent concerns and escalating patients to providers or emergency services when clinically indicated

  • Coordinating care with physicians, behavioral health teams, pharmacies, and other departments to promote continuity across the patient's care journey

  • Scheduling urgent, same-day, and follow-up appointments based on clinical assessment and provider recommendations

  • Coordinating prior authorizations and escalating denials or required clinical changes to providers for further review

  • Documenting triage encounters accurately, thoroughly, and promptly within the electronic medical record

  • Providing direct clinical nursing support within ambulatory and physiatry clinics

  • Assisting with clinical care during botulinum toxin (Botox) injection procedures within the physiatry clinic

  • Providing clinical oversight and guidance to medical assistants and other support staff as needed

  • Maintaining clear and compassionate communication with patients and families through both telephone and in-person interactions

This role might be for you if:

  • You enjoy combining hands-on ambulatory nursing with telephonic triage and care coordination

  • You are comfortable caring for children with a wide variety of complex medical needs

  • Strong nursing judgment allows you to assess symptoms, determine urgency, and identify the appropriate next level of care

  • You can recognize when a patient's condition requires immediate escalation while remaining calm and focused

  • You communicate clinical information clearly and compassionately to patients and families

  • You enjoy coordinating across providers, behavioral health teams, pharmacies, and other healthcare partners to keep care moving forward

  • You are comfortable managing multiple patient needs and competing priorities within a busy ambulatory environment

  • You value accurate documentation and understand its importance in maintaining continuity and patient safety

  • You enjoy educating patients and families and helping them better understand their care and next steps

  • You bring a patient-centered, collaborative, and highly responsive approach to nursing practice

To be considered for this Ambulatory Care Registered Nurse opportunity, you must be a graduate of an accredited school of nursing and hold a current New Jersey Registered Nurse (RN) license. American Heart Association (AHA) BLS Provider Certification is required. A bachelor's degree in nursing is preferred, along with previous pediatric nursing or ambulatory care experience. The ideal candidate demonstrates strong clinical assessment, telephonic triage, critical thinking, care coordination, patient and family education, communication, organization, and electronic medical record documentation skills. Candidates must be comfortable working with children with a wide variety of complex medical needs, demonstrate a high degree of sensitivity when interacting with patients and families, and be able to manage multiple responsibilities simultaneously. This is a full-time, day shift opportunity working 40 hours per week in the Ambulatory Clinic in Hamilton.
