The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Summary:

The Community Health Worker will primarily be maintaining a presence in the community with patients to promote, motivate and improve healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.

A typical day as a Community Health Worker may look like:

• Meet patients in a clinic, hospital, ER, and where patients are located and conduct needs assessment using Social Determinants of Health Tool
• Schedule weekly follow-up calls and home visits to patients
• Help patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members lives
• Assist patients with organizing their records, making follow-up appointments with a Primary Care Physicians or Specialists
• Help patients with the tasks of getting medical care, and work on health goals by helping to plan healthy meals or finding time to exercise
• Help patient s access resources, including quality of care, childcare services, food, housing, transportation
• Maintain relationship with the Community Based Organization
• Able to travel and meet the members where they at; may include local Churches, homeless shelter, and members residence
• Duties, responsibilities and activities may change at any time with or without notice

To be considered for the role, you must have:

• Required: High School Diploma or equivalent
